Self-Risk Assessment Survey: Are you at higher risk of heart disease?
 

Answer the questions below:

  1. Do you have a parent, brother or sister with a history of early heart disease?
  2. Do you have heart or circulatory problems?
  3. Have you ever had a stroke?
  4. Do you have diabetes?
  5. Do you smoke or are you exposed regularly to secondhand smoke?
  6. Has anyone in your family had high cholesterol?
  7. Have you ever had high blood pressure?
  8. Are you over 55?
  9. Do you eat foods that are high in saturated fat (such as fast foods, eggs, sweets or foods with lots of butter or cheese)?
  10. Are you overweight?
  11. Do you engage in an aerobic activity that is 20-30 minutes in duration, at least 3 times per week?
  12. Do you handle your current stress load well?
